Atopic Dermatitis (AD)

It is a chronic, relapsing, inflammatory, pruritic skin disease: “The itch that rashes” (1,2).

Scratching leads to loss of sleep and altered quality of life of the entire family. In children, AD interferes with quality of life more than other skin conditions (except scabies) (3).

Atopic Dermatitis affects up to 25% of children and 4–9% of adults worldwide.

In Australia, population prevalence is 20.3% at 1 year old and 16% at 4 years old. In GP’s consultation point prevalence (last 2 weeks) is 13.8% <4 years and 8.3% at 5-9 years (4).

80% of children develop AD by age 5 years with usual onset between 3–6 months of age. It does not always resolve during teenage years: 50% may persist after 10 years of evolution. Patients with skin of color tend to develop more frequent, severe and persistent eczema.

Atopic Dermatitis may be part of an atopic background with food allergy (early), asthma/wheezing (school age), allergic rhinitis (adolescent and adult). As the severity of AD increases, the frequency of asthma also rises (20-30% if mild, 70% if severe). Intense and early treatment of AD may prevent this Atopic March.

Atopic Dermatitis results from the interaction between the child and its environment.

The atopic background is genetically determined: if 1 parent is atopic the child has 40-50% risk of developing AD. If both parents are atopic this risk rises to 50-80%. There is a vicious cycle between alteration of skin BARRIER FUNCTION, INFLAMMATION and DYSBALANCE of the MICROBIOME. With time, prominent staphyloccus aureus becomes more virulent due to a biofilm formation.

Proven aggravating factors both during pregnancy and for neonate are active/passive smoking, antibiotic, urban living, pollutants, house dust mite. Prolonged EXCLUSIVE breastfeeding and post-partum depression represent risk factors too.

Multiple environmental factors (EXPOSOME) may trigger eczema crisis/flares: microbiological agents, stress, allergens (inhaled, food, contact), climate and irritants.

Prevention relies mostly on probiotics intake (not prebiotics) and daily emollients applications from birth with a minimum duration of 2 months (up to 24 months), mostly for neonates with a familial background of Atopy (5).

All treatment guidelines recommend regular/daily emollients applications, whatever the severity (6).

They may be applied alone or complementary to treatment (Corticosteroid-sparing effect), more quantity of emollient applied leading to less severity. Specific emollients for AD must trigger skin barrier function (dryness), inflammation (pruritus) and microbiome balance. Formulation/texture influences efficacy and compliance. Education of parents is key for better treatment and adherence: distinguish dry areas or chronic eczema (emollient) and inflammatory zones (topical steroids, others…).

Three studies were conducted under dermatological and paediatric control to evaluate the efficacy and tolerance of Stelatopia+ across different groups: children, adults, and pregnant women. The product was applied twice daily on clean, dry skin for 28 to 84 days and the subjects maintained a tolerance logbook.

Results:

• Adults (40 participants) 2023: 30 women and 10 men aged 18 to 65, with mild to moderate atopic dermatitis (SCORAD 25-30).
• Study methodology: Product Evaluation Questionnaire, Quality of life questionnaire specific to the participant and their family, developed by Expanscience. Clinical scores SCORAD, POEM, DLQI (Dermatology Life Quality Index), and IGA over 28 days.
• Pregnant Women (22 participants) 2023: Ages 20 to 36, with dry skin and a history of atopic dermatitis, between 15 to 28 weeks of pregnancy.
• Study methodology: Product Evaluation Questionnaire, clinical evaluations, quality of life questionnaires, and instrumental tests (Corneometer, TEWL) over 28 days.
